Besides request, natural persons have to enclose:

Legal persons have to enclose:

- the decision with the finality clause
- the power of attorney if the payment is made on the counsel's account.

Certification of documents for use abroad

Issuing Criminal Record Certificates
Certificates from the criminal record are issued in the Department for Criminal Records, Subsection for Administrative Tasks, Zagreb, Ulica grada Vukovara 49.
The office hours are 9 a.m. to 1 p.m. from Monday to Friday.
Request for Removal or Rehabilitation from the Criminal Record
The expungement of conviction from the criminal record is performed pursuant to the Article 85, paragraph 5, subparagraph 4 of the Criminal Code (Official Gazette 110/97, 27/98, 129/90, 51/01, 105/04, 84/05, 71/06, 110/07 and 152/08), and on condition that no criminal procedure is conducted against the person seeking the expungement of conviction, Article 473, paragraph 1 of the Criminal Procedure Act (Official Gazette 110/97, 27/98, 58/99, 112/99, 58/02, 143/02 and 115/06).
You can submit a claim for the expungement of conviction or rehabilitation from the criminal record via mail or personally to the Ministry of Justice, Criminal Law Directorate, Sector for clemency, register and indemnity, Department for Criminal Record, ulica grada Vukovara 49, and the decision will be delivered to your address within 30 days.
For removal from the criminal record a written request for removal from the criminal record and the Ministry's decision on rehabilitation should be forwarded to the Ministry of Interior, to the Department of Informing-Criminal Analitics, Ilica 335, pursuant to the Article 26, paragraph 1, subparagraph 2, Law on Police Affairs (OG 76/09). Timeline for enforcement of the procedure is 30 days. All other information can be obtained at +385 1 6321 702.
Appeal for clemency
Pursuant to the Article 5 of the Pardon Act, the appeal for clemency can be submitted after final judgement. The appeal is submitted to the Ministry of Justice of the Republic of Croatia (Department for clemency) or it can be submitted to the Writing office in the Ministry, Ulica grada Vukovara 49, Zagreb, or it can be delivered by mail on this address.
In the appeal it is necessary to specify the name of the court that adjudged the first instance judgement and judgement reference number, personal data of a convict, and data on family and social circumstances of the convict (for instance, number of children, especially minor, maintenance obligation, health condition of the convict and members of his family, if the family exercises incomes and how, as well as all those circumstances for which you consider that contribute to the convict).
The appeal for clemency has to be submitted by the convict personally, a spouse, and other persons cited in the Article 2, item 464 of the Criminal Procedure Act.
